What to include in a BDSM Application Form

This page provides essential information which Masters, Mistresses, Dominants may wish to ask potential slaves and subs before consideration for a relationship. A formal application form will be available here soon.

Anyone who has ever been to a play party will know just how diverse the BDSM scene is. Practitioners have vastly varied desires and needs, so finding someone to suit You can be tough. When meeting someone face to face, You can determine quite quickly whether he/she is worth consideration. Even though You may not end up being a good match down the track, at least You can get a general gist of what the person is like and what they are seeking. This is harder online, an avenue through which more and more partners are meeting each other.

Some people just rely on personal ads to meet someone whilst others take great care before they will accept anyone into their life. A number of Masters and Mistresses have set up their own website which includes Their own personal BDSM application form for consideration of potential slaves. Some are purely after sexual companions whilst others may just be service oriented contracts. Whatever Your fetish, screening applicants online via a form is an excellent means of finding that person to match Your wants and needs.

An application form doesn’t need to include a BDSM checklist, that can come later when you meet each other. The application can just be questions to find out about the person, their appearance, expectations, wants and needs. Most important of all, the questionnaire should be tailored to Your requirements. Here are some things which can be included in a personalized form:

Limit Your applicants

Be prepared that You may receive a lot of responses. In order to avoid sifting through a whole bunch of unsuitable contenders, be clear about what you are looking for. This is not disrespectful, You may have plenty of friends with these qualities but this doesn’t mean You have to accept them into a relationship. So, it’s perfectly ok to state something like “sorry but only female applicants between the ages of 21 and 40 please. No TS, TV or TG will be considered”.

Questions to Ask

You may want to ask the following questions in your form:

Personal details

  • Name, Email address
  • Age
  • Location
  • Sex (male, female, transgender, transsexual, transvestite)
  • Marital status (single, in a relationship, divorced, married, separated, married-cheating)
  • BDSM orientation (slave, sex-slave, sub, switch, Dom, Domme, sadist, masochist)
  • Sexual orientation (heterosexual, bisexual, lesbian, gay, pansexual)
  • Height, weight
  • Smoker/non-smoker
  • How many children?
  • Measurements (chest, waist, hips)
  • Upload/send Me recent photographs
  • Links to your website & online photos, if any
  • Highest level of education
  • About your occupation/current job
  • Religion

Compatibility issues

Query potential slaves about themselves and their personality, like:

  • Please describe yourself as a person
  • What’s your personality like?
  • Are you an extrovert (outgoing) or introvert (shy)?
  • Do you generally have a positive outlook on life?
  • Are you slutty?

About their intentions

Find out what they expect out of the arrangement and what they are willing to offer You. Here are some examples of what to ask:

  • Why do you want to serve Me?
  • What are you seeking from a Master/Mistress? What are your needs & desires?
  • If you are in a current relationship, please describe it.
  • Kinks
  • Hard limits
  • What commitments do you have have that will impact on your ability to serve now & in the future?

Health status

This is certainly a must have in any BDSM application form, but please note that your candidates’ answers may not be truthful. As a prudent sexually active person, the most responsible thing to do would be to have Your chosen slave medically tested for STD and STI’s before engaging in any unprotected intercourse with them. What to ask:

  • STD/STI status
  • Do you currently use drugs? If yes, which ones and how often?
  • What illegal drugs have you used in the past if any?
  • Do you drink alcohol? How much and how often?
  • Do you have any medical conditions or concerns?
  • Are there any physical limitations which would affect your ability to serve?
  • Prescribed medication
  • Any allergies? Phobias? Other issues?
  • Have you ever been diagnosed with depression, schizophrenia, bi polar, psychosis, anxiety disorders or neurosis? If so, did you/are you receiving any counseling or psychological treatment for same?

BDSM History and involvement

  • How long have you been in the lifestyle?
  • Have you ever been owned? If so, for how long?
  • If yes, may I contact your Dominant for a reference?
  • Are you a member of any BDSM organizations or clubs?
  • Have you/do you attend events, workshops?
  • What BDSM books have you read & like (fiction/non-fiction)?

Additional info

In addition to completing Your questionnaire, the interviewee might wish to add further information. Whatever format used for your application form, be sure to allow them space to do this. You might miss out on some essential details by only letting them answer your questions. You want to avoid a conflict of needs, expectations or personality as much as you can.

Give Your applicants clear instructions on submitting the form. Also, let them know when they should expect an answer from You.
